Het wordt gebruikt\u00a0<strong>als aanvulling op een caloriearm dieet en meer lichaamsbeweging.<\/strong>. Het is\u00a0<strong>Niet bedoeld voor cosmetisch gewichtsverlies.<\/strong>\u00a0of voor kortdurend gebruik.<\/p>\n<h4>Hoe Het Werkt<\/h4>\n<p class=\"mb-2 last:mb-0\">Contrave combineert twee werkzame bestanddelen:<\/p>\n<ul>\n<li><strong>Naltrexon HCl<\/strong>\u00a0(8 mg per tablet): Een opio\u00efde receptorantagonist die de eetlust en de drang naar voedsel kan verminderen door de beloningssystemen in de hersenen te be\u00efnvloeden (bijvoorbeeld door de hedonische effecten van voedsel te blokkeren).<\/li>\n<li><strong>Bupropion HCl<\/strong>\u00a0(90 mg per tablet): Een noradrenaline-dopamine-heropnameremmer (NDRI) antidepressivum dat de eetlust onderdrukt en mogelijk het energieverbruik verhoogt.<\/li>\n<\/ul>\n<p class=\"mb-2 last:mb-0\">De formulering met verlengde afgifte biedt 24 uur bescherming. De gebruikelijke dosering begint met 1 tablet (8\/90 mg) eenmaal daags gedurende de eerste week, waarna deze wordt verhoogd tot een maximum van...\u00a0<strong>2 tabletten tweemaal daags<\/strong>\u00a0(totaal 32\/360 mg\/dag) tegen week 4, indien verdragen.<\/p>\n<p class=\"mb-2 last:mb-0\"><strong>Bewijs<\/strong>Klinische onderzoeken (bijv. COR-I- en COR-II-studies) toonden een gemiddeld gewichtsverlies van ongeveer 5% over 56 weken ten opzichte van placebo (4-5% van de respondenten behaalde een gewichtsverlies van \u22655%). Een meta-analyse uit 2020 in\u00a0<em>Recensies over obesitas<\/em>\u00a0De bescheiden werkzaamheid werd bevestigd (3-5 kg \u200b\u200bmeer dan bij placebo), maar de nadruk werd gelegd op integratie in de levensstijl.<\/p>\n<h4>Doseringvormen<\/h4>\n<ul>\n<li><strong>Kracht<\/strong>: 8 mg naltrexon \/ 90 mg bupropion per langwerpige, filmomhulde tablet (geel; bedrukt met \u201c8\/90\u201d).<\/li>\n<li><strong>Verpakking<\/strong>Flesjes met 60 of 120 tabletten (voorraad voor 28 of 56 dagen bij maximale dosering).<\/li>\n<li><strong>Beheer<\/strong>: Slik de tablet in zijn geheel door met water; neem hem in tijdens de maaltijd om misselijkheid te verminderen. Bij lever- of nierfunctiestoornissen is een dosisaanpassing nodig.<\/li>\n<li><strong>Geneesmiddelinteracties<\/strong>Sterke CYP2B6-remmers (bijv. ticlopidine) verhogen de bupropionspiegel; opio\u00efden\/dextromethorfan worden geblokkeerd door naltrexon.<\/li>\n<\/ul>\n<h4>Beschikbaarheid en kosten<\/h4>\n<ul>\n<li><strong>Alleen voor merken<\/strong>\u00a0(geen generiek geneesmiddel beschikbaar vanaf 2024).<\/li>\n<li>Amerikaanse detailhandel: ~1300\/maand zonder verzekering; met kortingskaarten van de fabrikant (Currax) kan het bedrag worden verlaagd tot 99.<\/li>\n<li>Wordt door veel verzekeraars vergoed voor in aanmerking komende pati\u00ebnten.<\/li>\n<\/ul>\n<p class=\"mb-2 last:mb-0\"><strong>Belangrijk<\/strong>Dit is algemene informatie gebaseerd op FDA-etikettering, voorschrijfinformatie (currax.com) en studies (bijv. NEJM 2010; JAMA 2015).\u00a0<strong>Raadpleeg een zorgverlener.<\/strong>\u00a0Voor persoonlijk advies, aangezien Contrave deelname aan een REMS-programma (Risk Evaluation and Mitigation Strategy) vereist vanwege het risico op epileptische aanvallen.
